In order to have your Private Hire Vehicle Licence registration amended to your private DVLA registration you are required to provide us with the following documents.

The V778 Retention Document, V750 Certificate of Entitlement with the online reference number or V5 document and Insurance Certificate showing the new registration has been transferred.

Please use the form below to apply for a private registration in an already licenced vehicle. This will be sent to Licensing Services.

Once received, you will be contacted via telephone to make the £37 payment and then emailed an appointment within 2 working days to attend our offices at Chapel Street, Bilston, Wolverhampton WV14 0PH to collect your replacement plates.
 
You must attend the appointment alone and bring with you your:

  • Log book
  • Private Hire insurance certificate
  • Private Hire driver badge
  • Start
  • Preview
  • Complete
