This survey has been designed to enable the Council and community organisations across the City to understand what influences the lifestyles of Wolverhampton residents. During the COVID-19 pandemic, the lifestyles of residents in Wolverhampton may have been severely impacted. Therefore, there is a need for a lifestyle survey to be conducted to enable us to understand any changes to the lifestyles of residents.
The information gained from this survey will help the Council and community organisations make well informed decisions around services and support that they offer to residents of Wolverhampton. This will mean that Wolverhampton residents receive better support for their health and wellbeing.
This survey is intended to be anonymous, however in order to administer the incentive vouchers offered, a valid email address will be required for digital vouchers, or the last 5 digits of the survey reference number found at the end of the survey will need to be presented at a specified collection point, to obtain a physical voucher. Failing to present either of those could result in the £5 voucher being refused. This information will be used for the sole purpose of administering the incentive vouchers and will be deleted within 10 working days following the completion of the survey period. All physical vouchers must be collected within 10 days of survey completion. All vouchers are valid until 31st July 2023.
The vouchers can be used at community shops across Wolverhampton and participating local businesses. A map of community shops can be viewed below.
This survey will close on the 14th February 2023 and findings will be made available in Spring 2023.
