What is the key purpose of your service?

The key purpose of our service is to improve the life chances and achieve the best outcome for looked after young people aged 14-18 years and care leavers by ensuring they have support to achieve their aspirations and support in meeting their health, education, training and housing needs.

What are your key outcomes for families?

The key outcomes are that young people will be supported to have their pathway plans reviewed to ensure that their needs are being met.  That young people are given advice to assist them to look after themselves which includes health education, employment and training.  Provide supported and semi -supported housing choices.

What are the typical activities that you do? 

Some of the typical activities we do with young people are to work with them to identify the kind of support they will need to develop their independence.

The team will provide a worker who will complete an assessment and use the information to determine the type of housing, education, employment, training the young person may need along with ensuring they achieve your aspirations in life.
