Do I need to complete the claim correctly?

Yes. It is important that your application is correctly completed.

Incomplete application forms will lead to delays or your claim not being fast tracked.

What do I do with my claim?

Bring your form and all the relevant information to:

The reception is open from 8.30am to 5pm Mondays to Thursdays and 8.30am to 4.30pm Fridays but to allow time for us to go through your claim we operate Fast Track 8.30 to 4.00 pm Monday to Thursday and until 3.30 on Fridays.
