What is a care assessment?
Assessment is the word used to describe the way that we gather information about you so we can learn about your day to day difficulties and needs. We can then let you know if we are able to help.
If you think you need help from us you, or someone on your behalf, must contact one of our Adult Social Care Assessment Teams.
We will look at your needs and how we can help you stay in your own home. If that is no longer possible we will look at what alternatives are available.
We will talk about your circumstances and about the people who care for you already. We may need to get information from other people who know you such as a doctor or nurse. We will ask your permission before we do this.
When you have had an assessment we will talk to you and your carers about what help may be available and we will agree with you which services suit you best. Some people will have needs that we cannot help with, but if we can help you, we will.

Care services can only be provided after an assessment is completed.
Am I eligible for support?
Lots of people ask us for help. We have to decide what help we can offer. But we do not have unlimited time and money - so we have to have rules about how quickly we see people and what help we can offer in different situations. We call these our eligibility criteria.

How long will I wait?
Unless it is an emergency, we will contact you within two days rom the day we receive your request.
If you are in hospital and need help at home when you are discharged we will make arrangements while you are in hospital to come and see you. We will keep you informed about the progress of the assessment.
Charges for care services at home
Home Support Services are arranged by Adult Social Care Services and provided by a range of organisations. The aim is to enable you to remain in your own home and be as independent as possible.
When we arrange home support services for you, you may be asked to pay towards the cost. This is a charge which will be based on an assessment of your financial circumstances.

We offer support to people to help them understand and deal with the financial implications of going into a care home.
