Application forms

Wolverhampton City Council will accept applications for Gambling Act 2005 premises licences from 21 May 2007.

The forms to apply for a Premises licence/Permit are below. These forms must be returned to the Licensing Authority relevant to the area in which your premises are situated.

You can also access the forms to be used to give notice of your application to Responsible Authorities and to the public. You must give notice to the public outside the premises and in a local newspaper.

Downloading a form

To download a form, right click the mouse button over the relevant application form link and choose 'Save target as....', you will then be able to to save the application form in the appropriate place on your computer and complete the form offline.

• Application for a Licensed Premises Gaming Machine Permit
• Transfer/Vary a Licensed Premises Gaming Machine Permit
• Notification of up to 2 gaming machines in licensed premises
