Adult Continuing Health Care

The Individual Care Team, based within the Clinical Commissioning Group (CCG) delivers the NHS Continuing Healthcare Service for people aged over 18 who are registered with a General Practitioner (GP) in Wolverhampton.

NHS Continuing Healthcare is a national process that ensures the NHS pays for the care that a person needs if they have very complex healthcare needs.

When you reach 16 years of age, the team arrange for a checklist to be completed, which is the first stage of the assessment process. If this assessment is positive, then once you reach your 17th birthday, a full assessment will be carried out by a multidisciplinary team. This is to make sure that the people who are most familiar with your needs, including your family or carer(s), are involved. A document called a Decision Support Tool will be used at this assessment and it is usually possible to give an indication at this time as to whether you would be eligible for continuing healthcare or not.
          
For people who are eligible for NHS Continuing Health Care, the team can commission care to meet their identified needs. They can also work with you and support organisations to develop a Personal Health Budget that will meet your specific needs. A Personal Health Budget gives you a more individualised care plan with more flexibility and control over healthcare you receive.
