Community Nursing Children's Service

The Children's Community Nursing team consists of specially trained children's nurses and support assistants. We provide expert nursing care and support to children and young people in their own home, community or at Penn School or Green Park School.

We help children and their families to manage:

  • Life-limiting or life-threatening conditions
  • Oxygen dependent babies
  • Complex conditions
  • Acute and short-term conditions
  • Long-term conditions
  • Childhood cancers
  • Nasogastric tube feeding (through the nose)
  • Gastrostomies (tube feeding through the tummy)
  • Palliative care and end-of-life care
  • Bereavement with appropriate support

Community Nurses provide specialist nursing care for children who have acute health problems. This is usually on a short-term basis following discharge from hospital and is requested by a GP or hospital Consultant.

Care may include:

  • Wound care dressings
  • Injections, infusions
  • Enemas

This care may be given at your home, school or at the Gem Centre, as appropriate. Our teams also provide support in two special schools: Green Park and Penn Hall. This service cares for children who have learning as well as physical disabilities. Special School Nurses are available during school hours.

The CCN team offer a Enuresis clinic (bedwetting) for some children that are referred by paediatric consultant only For information about this service, please call the Senior Matron for Children's Services in Community on tel: 01902 446296.

We deliver your service at home, in schools and in clinics at the Gem Centre which is fully accessible with wheelchair access.

All clinic locations are fully accessible on the ground floor, but if you have specific needs that we cannot meet in our usual locations, your Community Children's Nurse will arrange for your care to be given in a more suitable environment. If you have specific access needs, please contact the Community Children's Nursing team and they will make the necessary arrangements for you.

The Gem Centre is open from 8.30am to 5.00pm Monday to Friday. You can call the Senior Matron for Children's Services in Community on tel: 01902 446296. Children's Community Nurses will normally visit you at home at an agreed time.
